CCC Mission and Aims

 MISSION: To provide resources and coordinate services for care in the community.

AIMS: To –

Be a link between individuals with needs (or potential needs) and NFA Council

Ensure that the transition to co-caring and dependence be held gracefully

Hold custodianship and facilitate use of the ‘Community Care Fund’

Provide information and facilitate access to benefits; health and social care

Bring accommodation needs to community awareness and actively support community housing with a high degree of facilities for ‘varying needs’

Facilitate social care and support

Identify vulnerable individuals and facilitate nursing care

Provide information about, and monitor storage and use of, care equipment

Provide information/education on the Curriculum for the 4th Age (skills and resources relevant for the final stages of life)

Create a culture of mutual support through encouraging the establishment of Buddy Networks for individuals and strengthen those networks as and when required

Assist the NFA in holding a record for NFA members of information that facilitates care in the community (eg. Next of Kin, Power of Attorney, Executor of Will, Buddies, specific medical details)

Facilitate access and social integration of individuals with varying needs (such as car turning circle for disabled passengers at Universal hall, gentle KP, social activities and amenities)

DEFINITIONS: ‘

Care in the Community’ means providing services and support for people with a recognised need to enable them to achieve maximum independence and control over their own lives whilst living in their own home or in a homely setting in the community.

Recognised need means problems related to illness; disability; or ageing. We do not limit our services to a group defined by their age but to individuals according to their needs

Co-caring – stage of some need and where there is the opportunity to give and receive support in a mutually beneficial way

Dependence – stage of intense care need
